Sevices Offered by Mathews Funeral Home
CHAPEL, CHURCH OR GRAVESIDE SERVICE PLAN
This service includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining certified death certificates and other forms and claims, preparation of necessary notices and coordination of service plans with parties involved in the final disposition of the deceased, embalming, chapel, church or graveside service, one night of visitation, use of automotive equipment including hearse, lead car, limousines for family and pallbearers and use of flower van within a 60 mile radius, register book, acknowledgment cards and memorial folders. This does not include casket, burial vault, opening and closing, cemetery space or obituary charges.
IMMEDIATE BURIAL
This service includes local removal, care of remains, basic services of staff (as mentioned above), authorizations and use of hearse to local area cemetery. This does not include funeral ceremony, visitation cemetery charges, casket, vault, opening and closing or obituary charges.
TRADITIONAL CREMATION PLAN
This service includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining certified death certificates and other forms and claims, preparation of necessary notices and coordination of service plans with parties involved in the final disposition of the deceased, removal of deceased to funeral home within a 60 mile radius, embalming and/or sanitary care of the remains, dressing, cosmetology and casketing of the deceased, use of facilities for visitation for one night, use of chapel for service, cremation fee, automotive equipment including hearse, lead car, limousines for family and pallbearers and use of flower van within a 60 mile radius. This does not include casket, permanent urn, memorial register, acknowledgement cards or obituary charges.
CREMATION WITH MEMORIAL SERVICE
This service includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining certified death certificates and other forms and claims, preparation of necessary notices and coordination of service plans with parties involved in the final disposition of the deceased, removal of deceased to funeral home within a 60 mile radius, use of chapel for the service, cremation fee and automotive equipment including limousine for family and pallbearers. This plan does not include casket, permanent urn, memorial register, acknowledgement cards or obituary charges.
DIRECT CREMATION
This service includes the minimum services of the funeral director and staff to assist in obtaining and filing legal documents, removal of the deceased from the place of death to funeral home within a 60 mile radius, crematory charge, minimum alternative container and minimum temporary urn. This service does not include casket, permanent urn, embalming or other preparation of the deceased, refrigeration, viewing or use of the facilities for viewing, funeral service, memorial register, acknowledgment cards or obituary charges.
FORWARDING REMAINS TO ANOTHER FUNERAL HOME
This service includes the minimum services of the funeral director and staff and use of the minimum facilities, obtaining and filing legal documents and other paper work necessary to complete the transferring of remains to another funeral home. This does not include casket, other preparation of remains, dressing, or any kind of cosmetology or obituary charges.
RECEIVING REMAINS FROM ANOTHER FUNERAL HOME
This service includes the minimum services of the funeral director and staff, use of the facilities for graveside service. This does not include casket, vault, cemetery charges, memorial register, acknowledgment cards or obituary charges.
